Who Is Eligible to Become a Surrogate?

Before medical testing begins, applicants must meet basic eligibility criteria, often guided by standards from the American Society for Reproductive Medicine (ASRM). Generally, candidates must:

  • Be between 21 and 45 years old

  • Have a BMI under 36

  • Be a non-smoker and drug-free

  • Have given birth to at least one child without major complications

  • Live with their child(ren)

  • Have no more than five vaginal deliveries and no more than three C-sections

  • Be financially stable and supported at home

  • Have no felony convictions (applies to both the surrogate and her partner)

What Does Medical Screening Include?

Medical screenings take place at the intended parents’ chosen IVF clinic. While procedures may vary slightly, most screenings involve:

  • Medical history review and physical exam

  • Pap smear and cervical swabs for infections such as chlamydia and gonorrhea

  • Blood tests for HIV, Hepatitis B & C, syphilis, HTLV-1, hormone levels (thyroid, prolactin), and blood type compatibility

  • Urine drug testing to ensure a drug-free environment for pregnancy

  • Ultrasound to examine the uterus

  • In some cases, a hysteroscopy to evaluate the uterus, cervix, and lining

All of these steps are designed to make sure the surrogate is healthy and that the pregnancy can move forward safely.

Psychological Evaluation

Just as important as medical testing, a psychological evaluation helps determine if a surrogate is emotionally prepared for the journey. This assessment is usually performed by a licensed therapist or social worker. Topics may include:

  • The strength of her support system during pregnancy and recovery

Drug and Substance Testing

Moving Forward After Screening
